Are you registered members at Risen Christ?  If your child will be receiving a Sacrament then you must be a registered parishioner or obtain permission from your parish.

I acknowledge I will obtain written permission from my home parish to obtain sacraments at Risen Christ Catholic Community.

If you are registering your child for a Sacrament Prep class please submit your child's Baptism Certificate to complete the registration process.  This is required before your child starts class.  By writing your name you acknowledge that you will provide the Baptism Certificate to Risen Christ.

Adult Volunteer

Would you be interested in learning more about volunteering with the children/youth? The adult volunteer programs are essential to the success of our Faith Formation programs and your support and assistance make this ministry possible.

In the event of an emergency, I hereby give permission to the staff and volunteers of Risen Christ Catholic Community to seek emergency medical transport and/or treatment for my child(ren) named above. I will be responsible for all costs incurred. I wish to be advised before further care is given by the hospital or doctor.

Consent and Liability Waiver

In the event of any accident or injury, I agree on behalf of myself, my child(ren) named above or our heirs, successors, and assigns, to hereby release and discharge the Diocese of Boise, its constituent organizations, including but not limited to Risen Christ Catholic Community and their officers, agents and employees from any and all claims for personal injuries or property damage that he/she may suffer as a result of his/her participation in the activity described above, whether or not such injuries or damages are caused by the negligence (active or passive) of any of the entities and individuals named above. Effective for all Faith Formation activities from September 2025 to September 2026.

PreK-5th Grade (With First Eucharist) = $100.00

Grades 6th-12th (No Confirmation) = $60.00

Sacrament Prep for OCIC (all ages - Baptism, First Eucharist and/or Confirmation) = $100.00

Active Parishioner

As a registered member of Risen Christ Parish I will regularly attend Mass on Sundays and Holy Days of Obligation at Risen Christ Catholic Community.

At least one adult member of the household wil be committed and active in at least one parish ministry.

I reaffirm my commitment to biblical tithing with a goal of 5% of gross income to the parish, 1% annual diocesan appeal and 4% of gross income to other charities.

I affirm my commitment to be a good example through my actions. I ask the support of my parish in nurturing my faith as I strive to form my child as a disciple through Catholic education. I pledge my cooperation with the parish and its ministries, and will make every effort to supervise my childs commitment as well.
